Connecting Caregivers with Services through Physicians

Making the Link: Connecting Caregivers with Services through Physicians is designed to strengthen the ability of Area Agencies on Aging (AAA) and Title VI Native American aging programs to serve family caregivers through a local, state and national campaign that will directly involve physicians in identifying caregivers and referring them to National Family Caregiver Support Program (NFSCP) services. Making the Link is supported in part by a grant from the U.S. Administration on Aging.

National Family Caregiver Support Program

Professionals & Providers This section of the National Family Caregiver Support Program webpage is designed specifically for professionals and providers who work with and serve caregivers of older adults (age 60 years and older), and grandparents and relative caregivers of children not more than 18 years of age, including grandparents who are sole caregivers of children and those individuals who are affected by mental retardation or who have developmental disabilities.
