Person-Centered Plan (PCP):
A plan developed with and for a person with disabilities that describes the strengths, preferences, needs and the dreams of the individual and the supports and services required and desired.Personal and home health care

An individual or organization authorized by law to serve as a guardian or conservator, usually for incapacitated individuals who have no one else to serve as their guardian or conservator. In Utah, the public guardian is the Office of Public Guardian (OPG).
